Patient Forms
If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
For your convenience we accept Visa, MasterCard, American Express and Discover. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance.
If you have questions regarding your account, please contact us at Rutledge Dental (520) 575-9449. Many times, a simple telephone call will clear any misunderstandings. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. Most insurance companies will respond within four to six weeks. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We accept Care Credit and Lending Tree third party finance companies in order to assist you.

We are in network providers for Delta Dental and Cigna. If you do not have dental insurance please check out our In-Office Dental Plan.
